This year we are surveying behavioral health agencies, within the State of Maryland, about the use or non-use of tobacco substances, policies and procedures, as well as preventative methods.  Since 1971, this information has been used by local, state, and national agencies for planning and providing treatment and prevention programs. We are only interested in the combined responses from all people/ agencies, not just one person’s answers. The information will be tabulated electronically, therefore, we will not have access to individual responses.  While the survey/interview has some personal questions, federal law keeps your answers private. We hope that protecting your privacy will help you to give accurate answers. You can quit the interview at any time and you can refuse to answer any questions.
